Intake & Assessment Case Worker

 

Summary

This position is a temporary, full-time, 12-month contract (maternity leave coverage)

Location: Cobourg, ON

Anticipated Start Date: December 2nd, 2024

Hours: 37.5/week

Salary: $24.00 - $28.00/hour

Reports to: Clinical Manager

Approval date: October 2024

Closing date: November 18th, 2024

Purpose of Position

The Intake and Assessment Case Worker position works in coordination with the Clinical Manager and care team to coordinate access to programming and services offered at Rebound Child & Youth Services. This position is responsible for intakes, assessment of client needs, and balancing caseload amongst the front-line staff. The successful candidate will also maintain their own caseload of clients requiring skill-building support.


Roles & Responsibilities

  • Receive and follow up on external referrals to programs and services from caregivers, community agencies, and internally from Rebound programs.

  • Conduct initial intakes and needs assessments with new clients and their families.

  • Based on intake and assessment, present recommendations for appropriate programming based on Rebound’s service model.

  • Assign caseloads to team members based on client needs, acuity levels, and staff capacity.

  • Maintain waitlists for Rebound programs and services and make recommendations to those departments to implement groups.

  • Provide flexible supportive services throughout the County of Northumberland to eligible children, youth, and their families in need of mental health/skill-building support services, which may include Triple P, Emotion Self-Regulation groups, 1:1 skill-building, or other programs as assigned by the Executive Director or designate.

  • Coordinate providing services to clients through linkages to mental health professionals, referral services, outreach services, and other community supports utilizing established protocols, as appropriate.

  • Monitoring client flow through service, in collaboration with Clinical Manager and appropriate staff member.


Administrative Responsibilities

  • Coordinate/maintain client files and requests for service delivery.

  • Organization and maintenance of filing system.

  • Perform clerical tasks/telephone communication as required.

  • Database entry and recording of information for referred and participating clients, including for purposes of program evaluation, assessment of needs and statistical reporting.

  • Maintain accurate program statistics.


Organizational & Team Support

  • Participate in community committees as arranged with the Executive Director.

  • Participate in team meetings and Rebound events as required.

  • Provide backup and support to team members as appropriate.

  • Understand and comply with Rebound Health and Safety Programs and AODA.

  • Contribute to the smooth operation of all Rebound activities.

  • Contribute to maintaining a healthy work environment.

  • Other duties as assigned and/or requested.


Working Conditions

  • Flexibility in work hours, including ability to work weekends/evenings for purposes of program delivery and attendance at activities and events where necessary.

  • Flexibility to work from home via virtual platforms as necessary and agreed upon with clinical manager.

  • Driving/travelling to rural locations within Northumberland County to provide services within Rebound’s community hub and other events.

  • Requires extensive keyboarding on different devices.

  • Requires working in shared open-concept office space, in a busy environment with frequent distractions.


Qualifications, Experience & Requirements

  • Post-secondary education diploma or degree(BSW or SSW) or equivalent experience in a related field (minimum 3-5 years).

  • Valid Ontario G Class Drivers’ License & reliable transportation, with minimum $2,000,000 liability coverage

  • Satisfactory Criminal Record Check including Vulnerable Sector Search.

  • Demonstrated knowledge and expertise in the field of community-based mental health services, with experience working with children, youth, and families.

  • Knowledge of relevant legislation pertaining to children and family services/children’s mental health services.

  • Excellent oral and written communication skills.

  • Ability to develop collaborative working relationships in an inter-professional and inter-organizational environment.

  • Working knowledge of community resources and pertinent service providers an asset.

  • Effective time-management and prioritization skills to handle the multi-faceted nature of the role.
